You may have a specific implant size and type in mind because it looks good on someone you know. However, the same implant options can produce completely different results in 2 different women for a variety of reasons, including:
Both types of implant materials are FDA-approved and have their own advantages. The best choice of implant depends on the needs of the patient.
Saline implants are filled with a sterile saltwater solution. Surgeons fill these implants after they’re placed in the implant pocket they create. This allows them to use a very small incision. In the unlikely case of a shell defect, the saline filling is harmless and is naturally absorbed by the body.
Saline implants are generally a bit less expensive than silicone gel-filled implants, although the price can vary depending on a number of factors. Saline implants are approved by the FDA for use in patients 18 and older.
Silicone implants are filled with cohesive silicone gel. Highly cohesive silicone implants are sometimes called “gummy bear” implants because their gel filling has a consistency similar to the gummy candy. Many people believe silicone gel implants create a more natural-looking appearance and feel.
In case of a shell defect, the cohesive silicone filling remains inside the breast capsule until it is surgically removed. Silicone implants have been approved for use in patients 22 and older.
Implants come in 3 projections, or profiles: high, moderate, and low. Just as different-sized implants suit different women, each profile may be a better fit depending on a patient’s goals. You can “try on” implants of different sizes and profiles during your consultation.
High-profile implants offer the most projection, which means they stick out farthest from the body. Because high-profile implants have a conical shape, they often look more proportionate on women with narrow frames.
Moderate-profile implants offer a medium amount of projection. Their rounded look can provide a significantly increased amount of breast volume, creating a fuller, natural-looking appearance for women with both narrow and wide frames.
Low-profile implants offer the least amount of projection but more fullness around the diameter of the breast. Low-profile implants offer a natural-looking result for patients with wide frames.
Instead of using bra cup sizes, breast implants are measured in volume by cubic centimeters (cc). As many women know, commercial bra sizes can vary greatly from brand to brand. Using cubic centimeters allows for a much more precise, reliable measurement. Having a desired cup size in mind is a good starting point, though.
Although it’s common for prospective patients to look to celebrities and other people as inspiration for their own breast augmentations, it’s important to remember that each body is unique. What looks good on one woman may look completely different on another woman of a different height, weight, or body type. Scarring is often a concern among cosmetic surgery patients. Though scars are inevitable in any surgery, the modern techniques we use at Synergy Plastic Surgery can greatly reduce scars’ appearance. Our surgeons are certified in the use of the Keller Funnel™, a device that enables them to safely insert implants using very small incisions. Inframammary incisions are made along the breast crease, where the breast meets the torso. Because this area is naturally concealed, an inframammary scar is well hidden in clothing and swimwear. The length of this type of incision depends on the specifics of the surgery, but is usually about 1 ¼ inch across resulting in a very favorable scar in most patients.
Periareolar incisions are made around the areola, which is the area of dark skin surrounding the nipple. Though this incision is made directly on the breast, the resulting scar often blends well with the darker skin of the areola, eventually fading enough to become nearly invisible. This incision is best suited for women with larger areolas, although it can also benefit women with smaller areolas who are receiving saline implants.
Transaxillary incisions are used less commonly and are made in the armpit. Many patients appreciate that this incision leaves the breast completely free of scars. However, because of its remote location, this incision provides the surgeon with less control and the scar can be quite visible in swimsuits and tank tops.
Breast implants can be placed either under or over the chest muscle (pectoralis). Most of our patients choose the submuscular placement (under the muscle) because it can achieve a highly natural look, especially if they have little breast tissue to start with. Submuscular placement can have a short recovery period, and this placement comes with a slightly lower risk of capsular contracture, a rare complication of breast augmentation.

Fat transfer breast augmentation, also called natural breast augmentation, adds fullness to the breasts using the patient’s own body fat. Our doctors typically advise patients to sleep on their backs for a few weeks after surgery to ensure favorable healing and an attractive, symmetrical shape. For women who are accustomed to sleeping on their sides or stomachs, this adjustment can be challenging.
It’s a good idea to begin the practice several days before surgery in order to train your body. Many patients also find it more comfortable to support their limbs with pillows. In most cases, yes. If you are concerned about breastfeeding, incisions should be made where the breasts connect to the chest (inframammary incisions.) That said, plenty of patients with nipple incisions (periareolar) are able to breastfeed. Regardless of where the incisions are made, it’s definitely safe to breastfeed with implants, which will not affect the milk.
It generally takes about 2 to 3 months for implants to soften and drop into a more natural-looking position. Most patients are thrilled with the results after 6 weeks, but your appearance will continue to improve. Massaging your implants for at least the first 3 to 6 months after surgery can help settle the implants sooner and keep the breast capsule open and soft.
Both saline and silicone breast implants are more durable than ever. Still, they are not designed to last forever. Each patient’s situation must be assessed on a case-by-case basis, but implants generally are meant to last 10 to 15 years. Each woman is different, but most surgeons in the U.S. who use modern techniques would advise you to have a submuscular augmentation. Implants above the muscle are sometimes more visible, and studies show they may have a higher risk for capsular contracture.
Breast implants can make performing and reading mammograms more technically difficult. However, placement of the implant underneath the pectoral muscle may interfere less with mammographic examination. Additionally, breast augmentation surgery is so common that virtually all radiologists are experienced at reading mammograms for patients who have implants.
If you’ve been researching breast augmentation, you may know that breast implants are available with smooth or textured surfaces. Some patients and doctors prefer smooth implants because they feel a bit softer than their textured counterparts and move more freely within the breast capsule for a more natural effect. Textured implants do not move as freely because breast tissue adheres to the surface, which is a benefit for patients with teardrop-shaped implants as this can prevent them from rotating.
In the past, the medical community collectively believed that textured implants lowered a patient’s risk of capsular contracture, an uncommon complication that occurs when augmented breasts develop an excessive amount of scar tissue. However, today’s advanced techniques have allowed surgeons to lower the incidence of capsular contracture in other ways.
