Case 240

This 55-year-old female found Dr. Kerr after being referred by her general surgeon.  She was interested in discussing reconstruction after a mastectomy and what options where available for her.

Because her cancer was small and located a large distance from her nipple a nipple sparing mastectomy and breast reconstruction was possible.  This allows the patient to keep their own nipple and step the nipple reconstruction step.  This method is only suitable for the right patients with a favorable breast shape (C cup or smaller) and tumor size and position.

This 42-year-old female was referred to Dr. Kerr from her general surgeon to discuss breast reconstruction options after mastectomy.

 She underwent a bilateral mastectomy with immediate breast reconstruction using tissue expanders after discussing her options with Dr. Kerr in detail.  This is the most common type of breast reconstruction performed in the United States.

Her first surgery was combined with the mastectomy.  Dr. Kerr admitted to the hospital overnight.  Her subsequent operations included implant placement, nipple reconstruction, and fat grafting (used to camouflage the implant and make are more natural result, also give the same result as liposuction to the areas we take the fat from).  These pictures show her final results after several months.

This 51-year-old female was referred to Dr. Kerr from her general surgeon to discuss breast reconstruction options. She had a right sided breast cancer and was found to be gene positive for BRCA.

She decided to have a bilateral mastectomy. Dr. Kerr spent a significant amount of time talking with the patient and helping her to understand her options.

These pictures show her after her complete breast reconstruction. This includes her expander placement, removal of implants, nipple reconstruction, and fat grafting. The fat her harvested for her fat grafting sessions has the benefit of giving the same results as liposuction to the areas we take it from which she was happy about.

This patient from near Austin, Texas came to Dr. Kerr at his Round Rock, Texas plastic surgery practice for breast reconstruction after being diagnosed with breast carcinoma insitu and deciding to undergo a mastectomy.  Understandably, this is very emotional time and Dr. Kerr and his staff pride themselves in helping to empower breast reconstruction patients while also teaching them about the options they have to restore their breasts after cancer surgery.

Her breast reconstruction involved three stages:  initially an expander was placed at the time of mastectomy, after expanding her breasts over about two months the expanders were removed and a silicone breast implant was placed, two months after this nipples were reconstructed and some fat was transferred from her abdomen into the breasts to make them more full. 

While this patient chose a heart shaped approach to areolar tattooing the results are still unique and beautiful.  Most patients opt for an areolar tattoo that most closely matched their native breast.

This patient from near Austin, Texas came to Dr. Kerr at his Round Rock, Texas plastic surgery practice for breast reconstruction after being diagnosed with breast cancer and learning that she was going to have to undergo a mastectomy.  During her consultation with Dr. Kerr spent about an hour teaching her about the various types of breast reconstruction surgeries that are available.  Dr. Kerr showed her where incisions are placed, and talked in detail about her recovery. He worked with her general surgeon to arrange a time for initial surgery and then he admitted her to the hospital for a few days after surgery.

Her breast reconstruction involved three stages which were completed in 5 months:  initially an expander was placed at the time of mastectomy, after expanding her right breast over about two months the expander was removed and a silicone breast implant was placed, two months after this her nipple was reconstructed and a lift was done on the left side for symmetry. 

Overall the size and shape of her reconstructed breasts fit her body very well and she is tremendously happy to have both her new natural appearing breasts and her feminine shape for the years ahead.

This 41-year-old female came to see Dr. Kerr to discuss breast reconstruction options.  She had been diagnosed with breast cancer and wanted to pursue reconstruction after a planned nipple sparing mastectomy in Austin, Texas.

Dr. Kerr met with her for about an hour on her initial visit.  He showed her pictures and discussed options.  She had a small breast and wanted to stay about the same size.  Given the size and location of her tumor a single stage reconstruction option (skipping the expander phase and placing an implant during the mastectomy) was possible.

This photo shows the patient several months after her single operation.  She is very satisfied with her outcome.  Her incision is about 3.5 inches long and well hidden in the fold of her breasts.  Her mastectomy was nipple sparing so she will not need additional nipple reconstruction.

Her breast reconstruction involved several stages:  initially an expander was placed at the time of mastectomy, after expanding her breasts over about two months the expanders were removed and a silicone breast implant was placed, two months after this nipples were reconstructed, and finally autologous fat transfer (AFT) was used to add fullness and shape to the upper breast using fat harvested via liposuction from the abdomen.  This is similar to the BRAVA technique also offered at synergy plastic surgery using autologous fat transfer (BRAVA-AFT) but also uses implants rather than fat grafting alone.

This 61-year-old female found Dr. Kerr on the web after looking for specialists to correct her prior breast reconstruction surgeries performed by other doctors.

Dr. Kerr met with her for about an hour initially.  They discussed what surgeries she had done before and then did a quick exam.  During the examination Dr. Kerr discussed how he could improve her symmetry and finish the reconstructive process by performing nipple reconstruction on the right breast.

During her surgery Dr. Kerr was able to reshape her breasts and exchange her breast implants for a better match side to side.  She has undergone additional procedure for full nipple reconstruction, scar revision, and fat grafting.  These photos show her final results and a dramatic improvement in symmetry and shape for both of her breasts.

This 45-year-old female was referred to Dr. Kerr from her general surgeon to discuss breast reconstruction options.  She had a left sided breast cancer that was going to require some additional skin excision than what is normally required for a mastectomy.

She decided to have a bilateral mastectomy.  Sometimes having a bilateral surgery can help create the most symmetric result but this is always an individual patient decision.  Dr. Kerr spent a significant amount of time talking with the patient and helping her to understand her options.
